In purchasing the puppy described, I agree to the following as indicated by my signature.

1. Danielle’s Doggie Den guarantees its puppies for 1 (one) year from the date of birth to be free of hereditary or congenital defects which causes death or a recommendation from a competent, licensed, practicing veterinarian that the dog be euthanized. All verifications must be presented in writing, on a veterinarian’s letterhead and signed by him/her. A necropsy report (at owner’s expense) is also required stating the defect. There are certain disorders that may be caused by “environmental” factors. I.E. allergies, thyroid dysfunction, demadectic mange, and other autoimmune disorders that are not covered by this guarantee.
2. Never, under any circumstances will the dog be placed in the humane society, pound or other facility (private or public) that practices experiments or euthanasia of animals.
3. Purchaser agrees to exercise normal care and protection in maintaining the puppy’s health. (a) Annual physical examinations by a competent, licensed and practicing veterinarian. (b) Annual vaccinations recommended by your veterinarian. (c) Annual screening for heartworm detection.
4. The puppy has been health certified by our veterinarian within 10 days whenever possible before departure, with no signs of illness or we have provided a Guarantee of Good Health. The puppy has been thoroughly wormed and vaccinated with at least the first series of shots.
5. Danielle’s Doggie Den does not guarantee the puppy’s size, weight, color or bite when full grown. It also does not guarantee its ability to reproduce.
6. Danielle’s Doggie Den exceeds the guidelines set forth by the PA State Lemon Law Act.
